- Classes will be filled as registrations are received.
  Children should be registered in religious education in the grade they are attending in school.
  A copy of your child's Baptismal Certificate is needed for those registering for kindergarten or first grade and for children new to the program.
  The certificates must accompany this registration form.

  Tuition: $200 per child and $60 for each additional child Grades K-8.
  *** PLEASE NOTE: Additional Sacramental Fee ($100/per child) for Grades 2 and 8 will apply.

  FLOCKNOTE
  
  The Office of Faith Formation utilizes FLOCKNOTE to inform parents of class cancellations due to inclement weather. In addition, it is used to distribute timely information on a regular basis via e-mails and text messages.
  
  You're automatically enrolled in Flocknote once your registration is processed.
  
  PARENT PARTICIPATION FORM REQUIRED
  In order to provide the students at Our Lady of the Mount with the best faith formation experience, participation on the part of the parents is welcome, needed and considered a blessing. Every parent is required to colunteer at least one hour of his/her time during the school year.
  1ST-8TH GRADE PARENTS (Coordinate or assist events) 6th-8TH GRADE PARENTS (Coordinate/Assist with Mini Retreat & Reception) DONATION OF FOOD (pasta, salad, bread, dessert) - we will contact you before the event BAKE SALE FUNDRAISER COORDINATORS/HELPERS (Meet with committee to choose and run two fundraisers during the year to support the program) COORDINATORS/HELPERS (family social, spiritual activities, movie night, living rosary, etc...)
